Lew MDI - Indications & Solutions
Lew MDI is a versatile and minimally invasive system aimed at solving certain implant situations in a simplified and cost and time effective manner both for the practitioner and the patient.
Lew MDI is not meant as a replacement for conventional implantology but as a complement. Main indications for the system are the edentulous mandible, the edentulous maxilla & Crown and Bridge.
Edentulous Mandible
A minimum of 4 implants should be placed in the lower jaw for a total denture restoration.
- We want to come a minimum of 7mm mesial to where we find the mental foramen, and use a Marker to define those points on both sides of the ridge, then mark the localization of the other two implants with even spaces.
- We also need to measure with a Ridge Mapping Caliper the width to make sure that the implant chosen is correct.
- A 1.5mm punch should be used first and is punched all the way through to the cortical bone.
- The pilot hole is created to give a minimum depth that allows for auto advancement of the implant into the bone. Park provides several drills to accommodate each preference.
- We are going to start with the Pilot drill and finally we’ll use the 1.1 mm drill because the tapered one is too wide, until we feel the drill drop through the cortical plate of bone.
- The implant can then be carried over to the patient’s mouth utilizing the vial cap on the implant. The implant is carefully inserted into the prepared starter hole with downward pressure to ensure proper engagement of bone.
- This should be continued with the multipurpose driver until finger resistance becomes noticeable.
- Once adequate resistance is obtained, the Park Dental Torque and Ratchet Wrench can be placed over the top of the Multipurpose Driver with the operator’s fingers under and supporting the patient’s lower jaw and the thumb placed over the top of the ratchet applying downward force down the long axis of the implant. The ratchet is then turned with no torque measurement. You continue placing the implant until you reach 35Ncm of resistance. Once the implant torque reaches 35Ncm and is fully seated, you’re done!
- Once the implants are fully seated, we will work on the prosthetics. To prevent locking of the denture over the tops of the implants, block out shims should be placed at this point.
- Place the Metal Housings in the patients mouth.
- The JUELL Cure Hard is used to incorporate with the metal housings into the denture. Do a reline. This usually takes from 7 to 9 minutes.
- The denture is then removed again, polished and trimmed, and then reinserted for any necessary adjustments.
Patients will appreciate your ability to give them back their ablity to speak, talk and laugh.
Edentulous Maxilla
A minimum of 6 implants should be placed in the upper jaw for a total denture restoration.
- Always make sure you avoid important anatomic landmarks, and use a Marker to define implant locations on the ridge.
- We also need to measure with a Ridge Mapping Caliper the width to make sure that the implant chosen is correct
- A 2.0 mm punch should be used first and is punched all the way through to the cortical bone.
- The pilot hole is created to give a minimum depth that allows for auto advancement of the implant into the bone. Park provides several drills to accommodate each preference.
- We are going to start with the Pilot drill and finally we’ll use the 2.0 mm drill, until we feel the drill drop through the cortical plate of bone.
- The implant can then be carried over to the patient’s mouth utilizing the vial cap on the implant. The implant is carefully inserted into the prepared starter hole with downward pressure to ensure proper engagement of bone.
- This should be continued with the multipurpose driver until finger resistance becomes noticeable.

- Once osseointegration has occurred you can remove the Soft Reline from the acrylic denture.
